Vial of L.I.F.E.
Lifesaving Information for Emergencies
Sponsored by Westwood Single Adults Ministry

The Vial for L.I.F.E. is a program which allows individuals to provide medical information in advance that can be used by emergency personnel in the case of an emergency. The Vial of L.I.F.E. is a medical history document that provides crucial personal and medical history in case of an emergency. The documents provide paramedics and firefighters with crucial information that will speak for you if you are unable to communicate in the event of an emergency. The Vial of L.I.F.E. is a nationwide program. All USA EMTs are trained to look for a Vial for L.I.F.E. although they will not hunt for it. They expect to find it on your refrigerator.

The Westwood Single Adults Ministry has teamed with the Cary EMS to sponsor this program to you and your families. Yes, we do include our guests.

The program consists of a vial (in our case, a plastic baggie), labeled with a Vial for L.I.F.E. sticker, inside which is a medical form which has been filled out stated the health status of each individual in your household as well as the current medications being taken. A magnet has been provided in each bag so that you can place on your refrigerator door, although you may need to tape it to your refrigerator so it will not fall off... One sticker goes on the outside of your bag so that it is visible to emergency medical staff when they enter your home to help you. The other is placed in the front window of your home so that the responding emergency personnel will know to look for the vial. It is also a good idea to place the following in the packet: a copy of an EKG, Living Will, DNR (Do Not Resuscitate order), and a picture of your family members with their name on it and attached to that person’s form.

You need only one baggie per family. You will have two forms in your packet. You will need to receive two additional forms for each member of your family. One completed form per person should go into the baggie; the other should be kept in a safe place so that you can keep it up to date. Please fill out the forms completely and print legibly. Note that the EMS team will not have time to sort through the baggie for the right person’s form. He/she will probably take the whole baggie and you will not get it back. Another reason to keep a back-up copy at all times for your next Vial for L.I.F.E. baggie.

Your Vial of L.I.F.E. will be invaluable to all of us, particularly anyone who has current health concerns; i.e. heart conditions, prior stroke, seizures; respiratory problems, allergies, etc. The Vial of L.I.F.E. program will only work if you keep your information up to date and complete. Additional forms may be obtained from this web site, the church media center, or by contacting one of the single adult members of our church.
